Introduction: Upper gastrointestinal bleeding is an important cause of morbidity, mortality and high medical costs, with peptic ulcer being the main cause of hemorrhage and H. pylori infection, its main etiology of ulcer. The histopathological study and the rapid urease test have a similar efficacy when it comes to establishing the diagnosis of H. pylori in ulceropeptic diseases. Objective: To determine the efficacy of the rapid urease test for the detection of H. pylori in upper gastrointestinal bleeding due to peptic ulcer, Hospital Antonio Lorena the Cusco, 2018. Material and Methods: Diagnostic test study type descriptive, prospective, crosssectional and correlational. Of quasi-experimental and observational design. Held at the Antonio Lorena Hospital in Cusco during the period from February 2018 to January 2019, with a population of 40 patients who met the inclusion and exclusion criteria. We proceeded to perform endoscopy for biopsy for histopathological study and rapid urease test. The data collection form is filled; the SPSS 23 version was used for information processing. In addition to Excel 2010 with a source of programmed data for evaluation of diagnostic tests. Results: The greatest number of patients comprises between 56 and 86 years of age, corresponding to 62.5%, a male predominance also with 62.5% and a female predominance of 37.5%. The clinical diagnosis of upper gastrointestinal bleeding occurred in 97.5% and endoscopic diagnosis in 100.0% of cases. The location of the peptic ulcers was gastric predominance with 55.0% followed by duodenal ulcer in 35.0%. In relation to the detection of H. pylori, it was the same in the histopathological report and the rapid urease test, I felt positive in 92.5% and negative in only 7.5%. Using the histopathological study as a gold test, a S = 97.3% was determined for the rapid urease test, E = 66.7%, PPV = 97.3%, VVN = 66.7%, LR (+) = 2.92, LR (-) = 0.04, diagnostic accuracy = 95.0%, diagnostic Odds ratio = 72.0, Kappa index = 0.640 and P = 0.000052. Conclusions: The rapid urease test and the histopathology have a similar diagnostic efficacy in the detection of H. pylori in patients with upper gastrointestinal bleeding due to peptic ulcer. The rapid result of the test could reveal the status of infection before hospital discharge, would allow early eradication treatment to start without loss of the patient and prevent rebleeding complications.
