Objective: To establish whether there is a relationship between knowledge about COPD and adherence to inhalation therapy in patients of the Antonio Lorena Hospital in Cusco, 2024. Method: This study is cross-sectional, correlational, and prospective. The sample consisted of 177 patients diagnosed with COPD who consented to participate in the study. Two validated questionnaires were included in the survey for the main variables, the COPD-Q and the TAI-10 items. The information was entered into an Excel database for subsequent processing and analysis using the statistical software STATA v.16. Hypothesis tests (Chi squared ) were used for the analysis of the independent variable and covariables. The logistic regression model was used to find the odds ratio (OR) of correlated variables. Results: The total sample consisted of 177 patients, of which 87 were surveyed by telephone and 90 were recruited from outpatient clinics. Females accounted for 62%, with a median age of 70 years (IQR = [63 to 74]). The predominant educational level was primary (49%). Polypharmacy was present in 32% of patients. The prevalence of poor adherence to inhalation therapy was 78.5%. Knowledge level about COPD was deficient in 23% and acceptable in 77% of respondents. Knowledge about COPD was related to adherence to inhalation therapy (p = 0.004), and it was found that individuals with acceptable knowledge of COPD are less likely to have poor adherence, OR 0.17, IC95% [0.04-0.173] adjusted for age and polypharmacy. Conclusions: Individuals with better knowledge about COPD are less likely to have poor adherence to inhalers. Age and polypharmacy were related to adherence to inhalation therapy. The prevalence of poor adherence to inhalation therapy was 78%. The knowledge of COPD patients about their disease is acceptable in 77% of patients.
