Objective: To determine the proximity between the root apex of the first lower molar and the bone vestibular surface in tomography of patients of the Dental Clinic of the Andean University of Cusco. Materials and methods: Nonexperimental, cross-sectional and descriptive study. A population of 121 dental Cone Beam computed tomography scans were archived in the database of the Dental Clinic "Luis Vallejos Santoni", for which observation was used as a technique and a data collection sheet as a tool. Results: We analyzed 121 tomography scans of patients between 18 and 50 years who attended the Dental Clinic "Luis Vallejos Santoni" of the Andean University of Cusco, in 2019. Where 72 (59.5%) were female and 49 (40.5%) male, according to the age of greater frequency was 18-30 years with 85 (70.2%). We found 210 lower molars of which 108 (51.4%) were from the left hemiarch and 102 (48.6%) from the right hemiarch. When reviewing the group distribution of 18-30 years have an average of 1.82mm, those of 31-40 years of 1.38mm and those of 41-50 years of 1.21mm, related to the female sex has an average of 1.72mm unlike the male sex with 1.62mm; and with regard to the distribution per hemiarch, the average for the left hemiarch is 1.82mm, for the right hemiarch 1.54mm, and in the distribution by root it has been obtained that the distal root has an average of 2.26mm unlike the mesial root with an average of 1.11mm, being obtained as an average of distance of 1.68mm. Conclusion: The proximity of the root apex of the first molars below the bone vestibular surface is 1.68mm.
