Introduction: Obesity is a systemic disease considered a silent epidemic (59), with the increase in obesity rates, related diseases have also increased (63). As women are a vulnerable population, we are facing the scenario of obese pregnant women that imply greater risks, and morbimortality (9). Objective: To determine if obesity is a risk factor for the development of maternal-perinatal complications in pregnant women treated at the Regional Hospital of Cusco, in the period 2021-2022. Materials and methods: This is an analytical, retrospective cohort study, with relational design. 184 pregnant women attended at the Regional Hospital of Cusco were included, selected by means of a nonprobabilistic sampling for convenience; of which 92 corresponded to the exposed group and 92 to the non-exposed group. A pregnant woman whose BMI was ≥30kg/m2, whose first prenatal check-up was performed in the first 10 weeks of gestation, and who had a diagnosis of a single, viable pregnancy, without pre-existing chronic diseases, was considered exposed. For the analysis, a tetrachoric evaluation was carried out that allows calculating the strength of association between the variables through the crude relative risk; and a logistic evaluation to determine the attributable relative risk. Results: It was found that 27.2% of the population was made up of women between 27 and 32 years of age, of which 50% presented some degree of obesity. Being grade I obesity the most reported (76%), followed by grade II (18.5%) and grade III (5.4%). An increase in the incidence of complications was demonstrated as the degree of obesity increased. Regarding the risk of developing complications; maternal obesity represented a 41.3% higher risk of developing maternal complications (p=0.000, RR=1.413, 95%CI [1.4812.218]), and a 15.2% higher risk of developing perinatal complications (p=0.001, RR=1.152, 95CI % [1057-1225]). Conclusions: It is concluded that obesity is a risk factor for the development of maternal-perinatal complications; Therefore, it is important to establish preventive measures in the community to reduce the incidence of this association.
