Introduction: Perineal traumas represent a multifactorial process that are associated to the nutritional deficit and the way of labor´s attention. Actually, perineal traumas are a high risk factor in the morbidity of the mother and the newborn. The objective of the study is to identify the risk factors associated with this pathology to carry out timely prevention. Methods: A retrospective and observational, case-control study was performed. The sample size was about 410 patients, divided in two groups: 205 cases (puerperal with perineal tear) and 205 controls (puerperal without a perineal tear). To see if the association is statistically significant, the values of the Chi-Square, the Odds Ratio and the Confidence Intervals were calculated, with a 95% level of confidence. Results: The most frequent type of tear in all of the study cases was grade I with 50.2%, into the risk factors that are associated to the perineal trauma was found: anemia (p=0.0005, OR: 2.02 ; IC 95% [1.36-3.01]), the long intergenic period (greater than 60 months) (p=0.0007, OR: 3.37; IC 95% [1.44-3.98]), precipitated delivery (p = 0.00, OR: 2.95, CI 95% [1.97 - 4.41]), performing more than 5 vaginal exams (p = 0.00, OR: 2.72, 95% CI [1.63 - 4.53], not doing episiotomy (p = 0.00, OR: 4.85, 95% CI [2.99 - 7.83]), the cephalic perimeter of the neonate from 33 cm to more (p = 0.00, OR: 5.49, 95% CI [3.28 - 9.20]), and delivery attended by the inmate (p = 0.00, OR: 2.44, 95% CI [1.63 - 3.67]). Conclusions: The study concluded that the statistically significant risk factors were: anemia, long intergenic period, precipitated delivery, performing more than 5 vaginal exams, not doing an episiotomy, a cephalic perimeter of the neonate more than 33 cm and delivery attended by the inmate
