Introduction: Depression has been linked to cognitive decline, being the main cause of disability and leading to poor social functioning, which would be worrying in vulnerable populations such as older adults. According to the 2017 Census, 28.1% of the population resides in the Sierra, geographically considered as an altitude zone. Due to the rugged geography of Peru, people living in these areas have less access to medical care and early diagnosis of diseases. This population is not immune to aging, this added to not having the facility to go to a health center would be considered more vulnerable to health problems including cognitive decline and depression, which determines these people as a subgroup of interest for the study of the relationship. Objective: To determine the association between depressive symptoms and cognitive decline in older adults living in high Andean communities in Peru. Materials and methods: Analytical cross-sectional study based on a secondary analysis using information from a primary cross-sectional study (AUNQUI-ANDES Study) on older adults living in a high Andean community in Peru. Cognitive impairment was considered the main dependent variable for the study. The main independent variable was depressive symptoms. To estimate the association between depressive symptoms and cognitive impairment, generalized linear models of the Poisson family with a logarithmic link function and robust variances were performed to estimate crude and adjusted prevalence ratios with their respective 95% confidence intervals (95% CI). Results: 276 older adults from the Totos community were analyzed. 59.8% were female, 40.2% were male, and the average age was 75 ± 8.6 years. 71.0% of these older adults had cognitive impairment and 56.9% had depressive symptoms. A relationship was found between the presence of depressive symptoms and cognitive impairment (p <0.001) in 79.6%. This relationship was maintained after adjusting for the variables sex, age, occupation, living alone, arterial hypertension, diabetes mellitus 2 and harmful habits (PRa = 1.18; 95% CI: 1.03 – 1.36; p = 0.018). The female sex showed a significant difference with the presence of cognitive impairment (p <0.001) compared to the male sex. Conclusions: Older adults from high Andean areas with depressive symptoms presented a higher proportion of cognitive impairment compared to older adults without these symptoms.
