Objective: To determine the relationship between initial platelet indices and the presence of acute cholecystitis in patients suspected of this condition. Methods: A convenience sample of 151 patients was selected, using an analytical cross-sectional study design. The included patients were adults admitted to the Regional Teaching Hospital of Trujillo, during the period from January 2019 to November 2023 with suspicion of acute lithiasic cholecystitis, excluding those with complications such as acute pancreatitis or gallbladder cancer. Data from clinical histories were collected for statistical analysis in SPSS version 28. Results: No significant differences were found in average age, sex, body mass index, arterial hypertension, type 2 diabetes mellitus, other comorbidities, and the duration of hospital stay between patients with and without acute cholecystitis. There were significant differences in leukocyte count, neutrophil levels, and PCR between the two groups. The mean platelet volume (MPV) was significantly lower in patients with acute cholecystitis. In addition, significant correlations were found between the MPV and other inflammatory markers. Conclusion: The results indicate that there is a significant relationship between initial platelet indices and the presence of acute cholecystitis, with the MPV being a potential useful indicator of inflammation in patients with this condition
