Background: During the COVID-19 pandemic, HIV/AIDS (Human immunodeficiency virus/Acquired immunodeficiency syndrome) healthcare programs were affected globally. The mortality trends in HIV/AIDS during the COVID-19 pandemic have not been explored in Peru. Objective: To describe age- adjusted HIV/AIDS related mortality during the period 2017-2019 (pre-pandemic) and associated with COVID-19 during the period 2020-2022 (pandemic) at national and subnational level. Materials and methods: Ecologic study based on HIV/AIDS related deaths registered in the National Informatic System of Deaths (SINADEF) from 2017 to 2022. Age-adjusted HIV/AIDS related mortality rates were annually estimated based on the direct method and described according to regions, natural areas and cause of death. Subnational sociodemographic variables were analyzed through linear mixed models. Results: Age-adjusted HIV/AIDS related mortality increased in 25,9% during the COVID-19 pandemic compared to pre-pandemic period. Loreto, Ucayali and Madre de Dios were the regions with the highest HIV/AIDS related mortality. Our linear mixed models showed that COVID- 19 pandemic and urban population were associated with increased mortality (1,65 (CI 95% 0,78, 2,51) y 0,12 (CI 95% 0,05, 0,19) respectively), while the average years of schooling were associated with lower mortality (-1,64 (CI 95% -3,02, - 0,30)). Conclusion: Peru showed an increasing pattern in HIV/AIDS related mortality from 2017 to 2022 with a higher increase during the pandemic COVID- 19.
