Introduction: Islet cell autoantibodies have been observed in type 1 diabetes (DM1), but may be detected in some cases of late onset type 2 diabetes (DM2). Currently there is an increase in DM2 in young people where studies suggest that there are differences between those where evidence of autoimmunity is found. Objectives: To determine the association of Islet cell autoantibodies and metabolic characteristics in adolescents with DM2 phenotype. Materials and methods: Cross-sectional study. Forty-one medical records of adolescents aged 12-17 years with a diagnosis of DM2 were reviewed at the Center for Diabetes, Obesity and Nutrition (CIDON) between the years 2018-2022. Data such as basal glucose, debut and control HbA1c, C-peptide, basal insulin as well as anti-GAD65, IA2 and antiinsulin autoantibody measures were recorded. Good metabolic control was assessed according to HbA1c< 7%. Two groups were compared: autoantibody (+) and autoantibody (-) with the metabolic characteristics of the studied population. Results: 39% of the population presented at least one Islet cell autoantibodies. Anti- GAD65 was the most frequent autoantibody reported with 24%. A significant association was found with C-peptide (p<0.001), HbA1c (p=0.041) and metabolic control (p=0.034). No association was found as basal glucose, insulin and HOMAIR. Conclusions: The presence of islet cell autoantibodies in adolescents with DM2 phenotype is associated with decreased C-Peptide values and metabolic control.
