The Direccion General de Medicamentos, Insumos y Drogas, regulatory authority in Peru is facing a challenge related to delays in the evaluation of applications for marketing authorization procedures. These delay, which exceeds the official deadlines established by local regulations, has been attributed to several factors, including a lack of trained personnel, divergent evaluation criteria, and duplication in assessments. The delay in the approval of major changes to medicines represents a problem for pharmaceutical industry because it limits their ability to implement product improvements in an agile and efficient manner. To improve the efficiency in evaluation times of regulatory authority, it is proposed to implement Reliance, an approach whereby regulatory authority could consider and give significant weight to assessments conducted by Reference Regulatory Authorities from other countries. The World Health Organization and the Latin American Federation of the Pharmaceutical Industry support this approach as an effective way to optimize resources and expertise. The aim of this work is to present a proposal for the application of Reliance for major changes in medicines registered in high health surveillance countries within the Peruvian regulatory context. To achieve this objective, a comprehensive analysis of the regulatory framework in Regulatory Authorities of Latin American countries such as Argentina, Brazil, and El Salvador was carried out, as well as of the recommendations and guidelines issued by the World Health Organization and the Latin American Federation of the Pharmaceutical Industry. Based on the experience of these countries and the recommendations of the mentioned international organizations, key aspects supporting the application of Reliance and the potential benefits this strategy can provide were identified. This research work succeeded in developing a robust proposal for the application of Reliance in Peru. This proposal considers key aspects for successful adoption, taking into account the necessary modifications in the current regulations.
