Background: Systemic lupus erythematosus (SLE) is a chronic disease characterized by abnormal activity of the immune system that presents various systemic manifestations such as depressive symptoms. Some studies have shown associations between proinflammatory cytokines and depressive symptoms in SLE; however, the existing literature is limited. Objective: To systematically summarize the existing literature in relation to cytokines and depressive symptoms in patients with SLE. Methods and Materials: A systematic review was carried out following the criteria of the PRISMA guideline and searches were carried out in PubMed, EMBASE, EBSCO, SciELO, ScienceDirect and Google Scholar of articles published until April 16, 2024. We included cross-sectional case-control design studies evaluating associations between cytokines and depressive symptoms in patients with SLE. Bias was assessed for each study using "Study Quality Assessment Tools" from the National Heart, Lung and Blood Institute (NHLBI). Results: 654 articles were identified, excluding 650 articles and including 4 articles. Cytokines evaluated included TNF-α, IL-6, IL-10, and IFN-I. In general, serum levels of TNF-α were associated with depressive symptoms in patients with SLE and a direct correlation was observed between TNF-α and the severity of depressive symptoms; however, there was no significant association with the other cytokines. Conclusion: Although there are studies that suggest an association between proinflammatory cytokines and depressive symptoms in SLE, the heterogeneity and scarcity of studies, as well as those with low statistical power, limit the generalization of these results.
