Grip strength has been identified as a risk predictor of general mortality and specifically cardio-metabolic morbidity and mortality. Studies carried out worldwide such as PURE, ACFIES and SALUS, in Latin America, agree that lower grip strength is associated with a worse metabolic profile. However, this risk may differ depending on ethnicity, geographic region, protein consumption, low birth weight, among others. The study carried out was descriptive, observational, cross-sectional in which 240 people were studied (80 Afro-Ecuadorians, 80 mestizos and 80 indigenous). The main exposure variable was ethnic group: Afro-Ecuadorians, mestizos and indigenous. The identification of the respective ethnicities was carried out taking as reference the national Census, where the ethnicity was identified according to three dimensions (common ancestors, attachment to culture and development of consciousness). Secondarily, physical activity, caloric intake, socioeconomic level, level of education, blood pressure, smoking, sex and age were evaluated. In addition, the commonly used anthropometric markers were considered: body mass index, abdominal circumference, waist-to-hip ratio, and waist-to-height ratio. Models were adjusted for these markers to observe variation and interaction with respect to the main exposure and outcome variables. The descriptive analysis calculated frequencies, for categorical variables, and measures of central tendency after evaluating normality in the case of numerical variables in each ethnic group separately (intragroup). Potential confounding factors (socioeconomic level, physical activity, etc.) were evaluated for each ethnic group respectively. For the bivariate analysis, the chi-square test was performed for categorical variables and the ANOVA test for numerical variables. Those variables that showed a significant difference were entered into a multivariate logistic regression model. The present study was carried out in Ibarra in the health units of Zuleta parish of Angochagua, where a predominantly indigenous population was found, Valle del Chota, parish of Ambuqui where the Afro-Ecuadorian population is and finally Ibarra where the mestizo population predominates. The determination of grip strength was different between the 3 ethnicities, mainly in the indigenous ethnic group in which more markers were found in relation to low grip strength, followed by the mestizo ethnic group and finally Afro-Ecuadorians. Furthermore, it was observed that low grip strength was found mostly in individuals with a higher BMI, where there was a greater accumulation of total and visceral body fat. The ethnic group that reflected the greatest correlation with decreased grip strength was the indigenous ethnic group, followed by the mestizo and finally the Afro-Ecuadorian.
