Introduction: The Work Capacity Index is an assessment tool that has been translated into different languages, its usefulness lies in allowing the early identification of a worker who has a high probability of reaching work incapacity. It also facilitates the implementation of prevention and intervention programs for workers at risk. The need to have validated instruments in the country within the area of occupational medicine, added to the benefits of the instrument itself, justifies the present research. Objective: To carry out the cultural adaptation and validation of the Work Capacity Index questionnaire. Methodology: Validation study of the instrument that was developed in a population of 314 health workers from two public hospitals in Ayacucho and Huancavelica. The process began with cultural adaptation, followed by obtaining evidence of content validity, internal structure, relationship with other variables and reliability. Results: The cultural adaptation was developed with a linguist expert, a group of experts and a pilot test who established changes for better comprehensibility. Evidence of content validity was developed with an expert committee whose Aiken method had a range between 0.77 and 1.00. Evidence of internal structure showed a unidimensional instrument with a variance of 47%. The criterion test validity of the overall ICT score with general health, r = 0.42, physical function, r = 0.40, bodily pain, r = 0.29 and social function, r = 0.37. The retest reached a value of 0.77. Conclusion: the Peruvian version of the ICT instrument demonstrated satisfactory psychometric properties.
