Cytomegalovirus infection is one of the most common postoperative complications in patients undergoing solid organ or hematopoietic stem cell transplants. This infection is one of the main causes of morbidity and mortality among these patients. The diagnosis of cytomegalovirus infection is performed using molecular techniques from blood samples. However, the use of saliva samples has shown significantly higher sensitivity compared to blood samples. In addition, saliva samples are easy to collect and are a non-invasive diagnostic alternative. Despite these advantages, to date it has not been evaluated whether the detection of viral particles in saliva represents an early indicator of infection or reactivation, and even less so, whether it can be used to monitor post-transplant infection. This study aimed to evaluate the usefulness of saliva samples in the diagnosis of reactivation of CMV infection. For this purpose, samples of liquid saliva, swab-collected saliva, and whole blood were analyzed using the commercial GeneProof Cytomegalovirus kit (CMV) PCR. The frequency of CMV-positive cases was 12.6%, 15.3%, and 12.1% in blood, liquid saliva, and swab-collected saliva, respectively. No significant differences in viral load were observed between the 3 sample types. A moderate correlation was observed between viral load in liquid saliva and swab-collected saliva (r = 0.3849, P = 0.0129). Similarly, a fair level of agreement was observed when positive rates were correlated between these samples (73.17%, P = 3 0.0564). The findings of this study suggest that saliva could be a useful diagnostic tool to detect and monitor CMV reactivation in transplant patients, particularly children, due to its simple and non-invasive collection process.
