Introduction: Current anemia diagnostic guidelines, which include corrections for altitude, may be overestimating its prevalence. There is a well-established relationship between socioeconomic status and anemia, which should persist at different altitudes. Objective: To evaluate the role of altitude as a modifier of effect in the association between anemia level and socioeconomic status through an analysis of the ENDES 2022. Materials and Methods: An analytical cross-sectional study was conducted using data from 20294 children aged 6 to 59 months from the ENDES 2022. Anemia was defined as hemoglobin levels adjusted for altitude below 11 g/dL. Poisson regression models were used to estimate prevalence ratios (PR) and 95% confidence intervals. Results: The prevalence of anemia is higher at higher altitudes, in poorer households, and in male children. Although the prevalence of childhood anemia decreases with increasing wealth level at lower altitudes, above 3500 meters above sea level, this decrease is not statistically significant. Conclusion: The study demonstrates that altitude is indeed a modifier of effect in the relationship between socioeconomic status and childhood anemia: while up to 3500 meters above sea level, a better socioeconomic level is associated with a lower prevalence of anemia, above that altitude, this association disappears. This would indicate that the adjustment of hemoglobin levels for that altitude may not be adequate, leading to overdiagnosis. It is recommended to review this adjustment.
