Background: Neonatal sepsis is a systemic disease of infectious origin with significant morbidity and mortality, especially in very low birth weight (VLBW) neonates. Care in the Neonatal Intensive Care Unit (NICU) entails significant costs, therefore, it is crucial to maintain updated control of them. In a hospital in Lima, a study was carried out in the NICU of the Dos de Mayo National Hospital and an average daily cost of S/. 3 231,341. Objective: Compare the cost of hospital stay between very low birth weight neonates with a confirmed diagnosis of late-onset sepsis versus neonates without late-onset sepsis hospitalized at the Cayetano Heredia Hospital between the years 2019-2022. Methods: Comparative study of two series. Series 1 was defined as neonates with confirmed late-onset sepsis and series 2 as neonates without late-onset sepsis. Matching was performed two to one, according to gestational age, birth weight and sex. The total cost per hospital stay was compared, based on a calculation made by Alvarado-Jaramillo, which was adjusted per year using a mathematical formula. The analysis was performed using Stata 17. Conclusion: VLBW neonates with confirmed late-onset neonatal sepsis had a higher cost and hospitalization time in relation to neonates without late-onset neonatal sepsis.
