Background. The uremic index (UI) reflects 24-hour urine creatinine clearance (CrCl) in patients with chronic kidney disease (CKD) G 1-4. As CKD progresses, changes in mineral and bone metabolism (MBM) are experienced. Objective. To evaluate the correlation between UI and alterations in the biochemical profile of the MMO in patients with CKD G 1-4. Material and Methods. The design was a correlation, analytical and cross-sectional study. 50 patients with CKD G1-4 treated on an outpatient basis in a Private Center, Lima, Peru between 2021 and 2022 were included. Spearman's measurement coefficient was used between the UI and the following variables of the MMO serum biochemical profile: 25(OH) vitamin D, total calcium and phosphorus; least squares exponential regression is used to evaluate the correlation between UI and serum intact PTH (iPTH). The normality test of the data was performed with the Shapiro-Wilk test. Results. It was shown that UI had a median of 4.25 mg/dl with an IQR of 2.4-9.9 mg/dl and presented a significant exponential correlation with serum iPTH (r2= 0.51, p < 0.000), this relationship persisted in patients with CrCl < 60 ml/min/1.73 m2 (r2= 0.7, p < 0.000) and in those under 70 years old (r2= 0.56, p < 0.000). However, the UI did not present correlation with total calcium (r= - 0.22, p= 0.13), with 25(OH) vitamin D (r= - 0.27, p= 0.06), with phosphorus (r=0.18, p= 0.19) nor with the CaxP product (r=0.06, p=0.65). Conclusion. The UI presents exponential correlation with serum iPTH in patients with CKD G1-4, mainly in patients with CrCl < 60 ml/min/1.73m2 and in those under 70 years old.
