Rheumatoid arthritis is a chronic disease that requires an effective diagnosis so that the doctor can differentiate it from other pathologies. In this sense, the Waaler-Rose test provides great specificity to detect RHEUMATOID FACTOR, an autoantibody present in the serum of people affected by seropositive rheumatoid arthritis. The conventional technique of this test uses sheep red blood cells sensitized with rabbit immunoglobulins, which will agglutinate in the presence of rheumatoid factor. In the present work, the use of human O “+” red blood cells instead of sheep is proposed to improve the specificity of the test in addition to reducing costs and simplifying its obtaining. Samples were taken from 50 patients with a diagnosis of RHEUMATOID ARTHRITIS and a positive Waaler-Rose test (conventional method) treated on an outpatient basis in the rheumatology clinic of the H.C.P.N.P during the months of August to October 1996. In addition, 50 samples from apparently healthy individuals were taken as controls. The result was a sensitivity of 96% and a specificity of 100% in the conventional Waaler-Rose test compared to a sensitivity and specificity of 100% using the modified Waaler-Rose technique. It should be noted that both techniques were standardized, so the only difference between them was the type of red blood cells used. It is recommended that different laboratories standardize this alternative technique, since it represents an improvement in sensitivity without loss of specificity and also greater accessibility when dealing with human red blood cells.
