Background: Hypervirulent Klebsiella pneumoniae can cause invasive infections in healthy and immunosuppressed patients. The overlap of hypervirulent phenotype with the carbapenem resistance phenotype is of particular importance and is the subject of epidemiological and genomic surveillance in East Asia, Europe, and North America. However, this surveillance is scarce in low and middle-income countries and there is a lack of information in South America. Objectives: Identify genetic biomarkers of hypervirulence and other genetic determinants of virulence in complete genomes of carbapenem-resistant K. pneumoniae clinical isolates from South America, as well as their clinical-epidemiological characteristics, and evaluate the association between the presence of virulence determinants and site of infection and mortality. Methods: A systematic search of genomic sequences was carried out from original studies and the NCBI genome database. The sequences were analyzed with the Kleborate tool, and the metadata was analyzed with R Studio. Absolute and relative frequencies were calculated, and the Chi square test was used to evaluate the association between virulence determinants and site of infection and mortality. Results: 1203 CRKp sequences were obtained from 8 South American countries. The biomarkers of hypervirulence found were aerobactin (1.1%), salmochelin (0.1%), rmpADC gene (<0.1%). Other virulence determinants found were yersiniabactin (53.2%), colibactin (24.7%), and the capsular serotypes K64 (13.9%), K2 (2.2%) and K1 (0.1%). The sequences had associated information: sex (39.7%), age (38.8%), comorbidity (31.8%) and outcome (8%). An association was found between a low virulence score and increased mortality and invasive disease. Conclusions: The study describes the co- occurrence of K. pneumoniae whole genome sequences with carbapenemases genes and biomarkers of hypervirulence. However, both biomarkers of hypervirulence and genetic determinants of virulence were found less frequently in these isolates compared to those in Asia and Europe. On the other hand, the presence of high-risk sequence types such as ST11, ST258, and ST437 across South America was identified, highlighting the importance of improving genomic surveillance in the region.
