Background: During the SARS-CoV-2 pandemic, the number of cases of Acute Respiratory Failure (ARF) due to COVID-19 increased, making it necessary to study the Oxygenation Index (OI) as a parameter associated with mortality in patients who required invasive mechanical ventilation. This allows for an assessment of the severity of hypoxemia in intubated patients. Objective: Determine the association of the Oxygenation Index, age, sex, comorbidities and prolonged stay in ICU with mortality in patients with Type 1 ARF due to SARS-CoV-2 on mechanical ventilation in the ICU of Hospital Nacional Arzobispo Loayza in 2021. Methods and Materials: Cross-sectional, analytical, retrospective study. A patient database with a diagnosis of Type 1 ARDS due to SARS-CoV-2 requiring mechanical ventilation in the HNAL ICU, collected from March to July 2021, was utilized. The sample consisted of 85 patients. Univariate, bivariate, and multivariate analysis of oxygenation index, age, gender, comorbidities, and prolonged ICU stay associated with mortality was conducted. Results: Mortality was 57,7%, with a mean age of 51 years, and 64,7% were male. Multivariate logistic regression analysis revealed an association with mortality for OI ≥ 15 (OR 4,4; 95% CI 1,2-16,1; p = 0,025) and comorbidity (OR 2,9; 95% CI 1,1-8,1; p = 0,040). Conclusions: The oxygenation index and the presence of comorbidity were associated with mortality in patients with Type 1 ARF due to SARS-CoV-2 on mechanical ventilation in the ICU of Hospital Nacional Arzobispo Loayza in 2021.
