Background: There is an overlap in the evaluation of lower urinary tract symptoms (LUTS) when the detrusor underactive (DU) and the bladder outlet obstruction (BOO) and there are no International Continence Society (ICS) criteria that precisely define DU, being necessary to use the urodynamic study. Objective: Describe the clinical and urodynamics characteristics in men whit DU and whit BOO. Material and methods: It is a series of retrospective, cross-sectional and exploratory cases between 2 groups of male patients with LUTS. Group 1 patients without BOO or unequivocal in the ICS nomogram, but with a diagnosis of DU in the Schäfer nomogram. Group 2 patients with BOO in the ICS nomogram. The clinical and urodynamic characteristics of both groups are described and compared; A logistic regression model was applied to identify which variables independently predict DU. Results: 95 patients were included, 36 with DU in Group 1 and 59 with BOO in Group 2, both groups did not differ in age and frequency of symptoms (IPSS). Among the clinical characteristics; age ≥ 56 years was related to the absence of DU [OR: 0.03 (0.01 – 0.26); p = 0.001], while the DM [OR: 4.84 (1.21 – 19.34); p = 0.025] and the DHIC [OR: 24.093 (6.66 – 87.10); p = 0.025] were related to the presence of DU. In relation to the urodynamic variables, the variables that best identified the presence of DU were; the WF ≤ 7 [OR: 23.10 (5.52 -96.67); p = 0.001] and the BCI ≤ 99 [OR: 6.93 (1.92 – 24.90); p = 0.003] Conclusion: It is concluded that the urodynamic variables WF ≤ 7 and BCI ≤ 99 were the ones that best described the presence of DU.
