Malaria has high relevance in our country. Despite progress, it is still a persistent problem in the Peruvian Amazon, where more than 80% of cases are due to Plasmodium vivax. However, in recent years, there has been a significant reduction in cases. This reflect a current scenario of residual malaria with heterogeneous transmission, characterized by the high occurrence of asymptomatic infections which contributes up to 73% of community malaria transmission in Peru. The present study analyzed the intra-host evolution of malaria symptoms and Plasmodium vivax genotypes in recurrent infections in a longitudinal study in 29 communities of the Peruvian Amazon. Our goal is to provide information on the factors associated with asymptomatic recurrences. To this aim, basic data on the parasite's individual, infection, and genotype (14 neutral microsatellites) were analyzed from a cohort of 302 individuals with malaria by P. vivax, treated with chloroquine + primaquine. Participants were also systematically monitored for up to 2 years. The 66% (198/302) of the participants experienced at least one recurrence over time. A total of 609 recurrences were reached, even having records of participants who experienced up to 11 recurrent episodes. Of the total recurrences, 65% (398/609) were asymptomatic. Regarding intra-host evolution, participants had an increased risk of developing episodes with symptoms (log-rank p<0.001) over time. However, individuals tended to become asymptomatic if they previously experienced symptomatic infections. Experiencing a recurrence with homologous parasites to the initial infection, and being an individual aged 6 to 60 years were associated with experiencing an asymptomatic recurrence. In the other hand, living in isolated communities reduced the risk of developing these infections. We conclude that the appearance of asymptomatic recurrences is associated with both the characteristics of the host and the parasite. The knowledge of these factors would be relevant for the development of more assertive interventions against residual malaria in the areas of the Amazon.
