Introduction: There are conflicting results regarding the association between SARS-CoV-2 viral load and risk of death. Clarifying this association could contribute to the management of patients with COVID-19. Methods: An observational, analytical, retrospective cohort study was conducted. The population includes adult, hospitalized and outpatient patients with and without symptoms diagnosed in the DIRESA Amazonas between August 2020 and August 2021. The standardized Ct values (indirect estimation of viral load) of the ORF1ab gene were divided by the median into low Ct and high Ct, the cut-off point was 28.4 for both patients with symptomatic COVID-19 and hospitalized with COVID-19. Survival was compiled from the MINSA Notiweb epidemiological database. The survival rate (Hazard) was estimated using a Cox regression adjusting predictor and confounding variables. Results: A total of 1,791 SARS-CoV-2 positive patients were identified by qRT-PCR, of which 1,693 were symptomatic, including 273 hospitalized. In patients with symptomatic COVID-19, there was no difference in the risk of death between patients with low and high Ct values (aHR=1.61; 95% CI, 0.97-2.67; p = 0.067), but an increased risk of death was found in patients hospitalized with COVID-19 who had low Ct values (Ct≤28.4), (aHR=1.82; 95% CI, 1.06-3.12; p = 0.030). Conclusions: The Ct value of SARS-CoV-2 (indirect estimation of viral load) in the adult population with symptomatic COVID-19 (hospitalized and outpatient) is not associated with survival from COVID-19, however, in hospitalized patients it is associated with the risk of death after adjusting for age, sex, occupational group, days of symptoms, comorbidities and epidemic dynamics.
