Endodontic procedural errors (EPE) are frequent as teeth move posteriorly. Determining the factors associated (FA) to the most frequent EPEs during endodontic treatment (ET) allows to reduce the risk of failures that entails to perform a non-surgical or surgical retreatment or to consider extracting the tooth. Different studies have been carried out using radiographic analysis and others using cone beam computed tomography (CBCT), the latter being the gold standard. The aim of this study was to evaluate FA to EPE requiring retreatment. A total of 96 CBCT, 122 teeth with TE and a total of 227 canals were analyzed in HOROS software. An association was found between the transport of canals with the type of tooth and periapical reaction; in addition, the omission of canals was associated with the type of tooth, calcifications, and type of root curvature; likewise, an association was found between the quality of obturation with the type of curvature and calcification; finally, fenestration was associated with periapical reaction. It is concluded that procedural errors were found to be associated with the factors studied in this research, and that CBCT is indispensable for the diagnosis of cases of endodontic retreatment.
