BACKGROUND: The percentage of early initiation of breastfeeding (EIB) globally varies between 30.8% and 60.3%. In Peru, the figure is less than 50%. Various sociodemographic, maternal and institutional factors have been reported that are related to the EIB, but little has been studied about the level of complexity of the health establishment and its relationship with the EIB. Objective: To identify the relationship between the level of complexity of the health facility where the birth was attended and the early initiation of breastfeeding. METHODS: A secondary analysis of the Demographic and Health Survey (DHS) in Peru 2019 was carried out. Women aged 15 to 49 years with a live birth, product of a single pregnancy, with vaginal delivery in an establishment of the Ministry of Health of Peru or EsSalud, within 24 months before interview were included. Poisson regression models were generated to assess the association of interest, reporting prevalence ratios (PR) and 95% confidence intervals (95% CI). RESULTS: The total sample was 3389 women. Women whose delivery was attended at the 1st level of care had a 9% greater probability of EIB, compared to women whose delivery was attended at a 2nd and 3rd level of care facility (RP 1.09 IC 95%: 1.03 a 1.15). CONCLUSIONS: Children born in 2nd and 3rd level care facilities experience less EIB and therefore hospitals could improve their EIB policies and practices.
