Introduction: The effective communication between health personnel and the patient in a clinical interview has been evaluated using the CICAA scale; however, it is not taken from the patient's perspective. Objective: Design and validate an instrument to measure the perception of patients about medical care in outpatient clinics based on the CICAA model in a first level care center of the Social Security Health. Materials and methods: Cross-sectional, prospective observational study. A questionnaire (28 questions) was designed from the CICAA model. Expert physicians carried out face and content validation, the questionnaire was applied to patients upon leaving the outpatient clinic of the Juan José Rodríguez Lazo Polyclinic after the medical consultation. A Microsoft Excel database was created for analysis. The STATA 17.0 program was used. Exploratory and reliability factor analysis was performed. Results: The content validation was carried out with the Aiken V test with an acceptable concordance value (>0.7). The instrument was applied collecting 345 surveys. The relevance of the exploratory factor analysis (EFA) was evaluated with Bartlett and KMO test. Then, the AFE was carried out by principal components and Varimax rotation, identifying 3 factors that explain 85.55% of the total variance. Reliability was good, Cronbach’s alpha on 0,91. The final instrument was called the CEER scale, the user's version. Conclusions: The CEER scale version for users is a valid and reliable instrument to evaluate two-way communication in the face-to-face clinical interview from the patient’s perspective.
