Background: Viruses are the most common cause of acute diarrheal disease in children under five years old, followed by bacteria and parasites. Since it is uncommon to test diarrheal samples for viral agents in the laboratory, immunochromatographic tests serve as valuable tools in promptly identifying the etiological agent, enabling swift provision of evidence-based care. Objective: to estimate the prevalence of viruses using immunochromatographic tests in diarrheal samples collected from children under five at the National Institute of Child Health between November 2019 and March 2020. Methods: Descriptive, cross-sectional analytical study. Fecal samples from under-five-year-old patients at the emergency and outpatient services were examined. Immunochromatographic tests for rotavirus and adenovirus were utilized, with prior verification of their accuracy. Associations between children's demographics and microbiological results were explored using bivariate analysis. Results: the prevalence of rotavirus and adenovirus was 10.4% (20/192) and 7.3% (14/192), respectively. Notably, attending the emergency department was associated with a higher likelihood of rotavirus positivity (p=0.043). Factors such as gender, age, fecal leukocyte count, Campylobacter spp detection, bacterial isolation, and the presence of parasites did not exhibit significant associations with viral detection. Conclusions: viruses emerge as the primary causative agents of acute diarrheal disease in children under five years old, surpassing bacteria and parasites in frequency. Given the infrequent screening for viral agents in routine laboratory practice, immunochromatographic tests prove crucial in ensuring swift and accurate diagnosis, facilitating timely and targeted evidence-based healthcare interventions.
