Introduction: During "El Niño Costero" (2017), there was an outbreak of leptospirosis. There are standard Faine criteria, which were later modified by adding variables such as the presence of rain and other laboratory tests. Objective: To determine the diagnostic assessment of the Faine criteria and their modification, in patients with suspected urban leptospirosis during the "El Niño Costero" phenomenon in a hospital in Chiclayo, 2017. Materials and methods: 83 epidemiological files of suspects were evaluated by leptospirosis. Those who had a positive gold standard test (microagglutination, MAT) were considered positive for the disease. Both Faine criteria were compared according to the gold standard, to obtain sensitivity, specificity, predictive values, and likelihood ratio. Results: There was a 41% positivity to MAT. Sensitivity and NPV had statistically significant differences, but it could be influenced by the fact that the score includes the MAT test. PPV, CP and ROC curve did not have statistically significant differences. Conclusions: The standard and modified Faine criteria have form and substance limitations when including the MAT within the score. The waiting time for the MAT result had an average of 24 days, and a maximum of 59 days. The results apply only to patients with suspected leptospirosis, with hospitalization criteria. Recommendations: It is suggested to reconsider and validate the Faine criteria culturally and locally, with a proportional weighting to the items and without including diagnostic aid tests.
