Fascioliasis is hyperendemic in Peru and it has non-specific symptoms in the community that makes it difficult to identify. The aim of this study was to determine clinical findings or combinations that allow the identification of chronic fascioliasis in children from the community and propose a diagnostic score. Methodology: Cross sectional - analytic study. Secondary data analysis of the study “Impact of Fascioliasis among Children in the Peruvian Highlands” from 3 to 16 years old in Cusco. Chronic fascioliasis was defined as presence of Fasciola hepatica eggs in feces. Negative infection was defined as absence of Fasciola eggs or other parasites in feces and negative Fasciola serology. Demographic, clinical and laboratory information were obtained (three stool specimens, blood sample for Fas2-ELISA). ROC curves and logistic regression were used to determine the sensitivity and specificity of symptoms and laboratory combinations to identify chronic fascioliasis. Results: 909 children were included, 162 with chronic fascioliasis and 747 negatives, being female (52%), mean age of 9.6 years (±3.6). Comparing children with chronic fascioliasis and those negative, no differences were found by sex (p=0.690) or age (p=0.082). In chronic fascioliasis were more frequent: right upper quadrant abdominal pain (p≤0.001), fatigue (p≤0.001), anorexia (p=0.006), vomiting (p=0.009), diarrhea (p=0.040); abnormal: hematocrit (p=0.038), hemoglobin (p=0.008), leucocytes (p=0.043), eosinophils (p≤0.001). Combination of right upper quadrant abdominal pain, fatigue and eosinophilia identified chronic fascioliasis with high sensitivity (85%) and specificity (92%). Conclusions: Children with chronic fascioliasis have characteristics that allow use the proposed score for their identification in endemic communities.
