Despite the existence of policies focused on reducing tuberculosis since the 1980s, adherence to treatment continues to be a significant challenge. In 2016, Peru registered a treatment abandonment rate of 6.8% for sensitive TB and 33.9% for MDR TB. Health personnel usually have practices outside of those strictly established; in most cases with the aim of generating greater trust with their patients and motivating them to attend regularly to receive treatment. This research uses Michael Lipsky's concept of Street Bureaucrats to identify those practices and their motive. This investigation analysis of 18 in-depth interviews with health personnel who are working on the Tuberculosis Strategy in the health centers of the micro network studied. The results shows that non-compliance with the requirements of the technical standard is not sufficient to identify discretionary practices. This is due to the design of the technical standard, the limited knowledge of it and the relevance of practice in professional training. The discretionary practices identified were a) giving treatment at home, b) collecting sputum tests from non-asymptomatic patients, c) collecting and delivering “aids” to patients, and d) working out of shift. All discretionary practices identified, except for collecting sputum tests from non-asymptomatic patients, were performed to improve patient adherence to treatment. The determinants of these practices must be understood from the understanding of the way of working the health strategy against TB: which is made up of the role of the person in charge of the health strategy, the availability of health staff, the goals of the strategy against TB, professional values, the learning of the norm and the way of working in the health center, and the perception of the norm as something interpretable.
