The aim of this study was to determine the effect of prenatal education (PE) on the initiation and maintenance of exclusive breastfeeding (EBF) and postpartum depression (PPD). A cohort study was conducted in 278 women, 152 who received PE and 126 who did not receive PE, distributed in public and private institutions in Quito, in which telephone interviews were conducted a month, at four and six months postpartum, to assess the effect of the PE on the initiation and maintenance of the EBF and on the presentation of the PPD. The independent effect of other intervening variables related to prenatal control and delivery care was also evaluated. A lower incidence density of EBF abandonment and PPD per month was found in those who received PE, which was 10.7 cases of EBF abandonment and 1.4 cases of PPD per 100 person - month of follow-up (RR: 0.722; 95% CI: 0.627 0.832, RR: 0.348; CI 0.190-0.635, respectively). PE has a protective effect for the maintenance and duration of EBF, as well as for PPD in the first postpartum month.
