Two epidemics affect our public health: we have a high incidence of multidrug resistant tuberculosis and overweight / obesity continues to increase. The immune system and the metabolic system work in homeostasis. Malnutrition can lead to changes in the pharmacokinetics of various treatments. Little is known about whether excessive nutrient increases can alter pharmacokinetics. We explored with two research questions whether overweight / obesity could cause resistance to retreatment and poor response to treatment as an adverse result of causing subtherapeutic doses of the drug and producing effects on the immune system. Study 1: Cross-sectional analysis of a tuberculosis cohort (2007-2014), of anthropometric tests and sensitivity tests at the baseline visit of patients with and without prior treatment. 3,734 new cases and 766 with a tuberculosis history were evaluated. Overweight / obesity was not associated with multidrug resistance in patients with a history of treatment, prevalence ratio (PR) 0.97 with a 95% confidence interval, 95% CI of 0.68-1.38. Study 2: Longitudinal analysis of the same cohort, from baseline anthropometric tests and sputum cultures of 2, 12 and 24 months of new cases of tuberculosis, to determine success or failure of treatment. 2,781 new cases with positive culture were evaluated at their baseline visit, 592 dropped out, 150 died and 2,039 ended the follow-up. We found no association between overweight / obesity and poor response to treatment in new cases of tuberculosis, PR: 0.79, 95% CI: 0.59-1.03.
