Background: The subarachnoid hemorrhage secondary to ruptured intracranial aneurysms carries a high mortality and morbidity. Especially, paraclinoid aneurysms located in complex anatomic locations of the skull base demand challenging microsurgical techniques and vast experience to optimize their treatment. In Peru, scarce studies have evaluated the clinical outcomes of patients who underwent microsurgery for paraclinoid aneurysms, which motivates us to research the neurological status during the follow-up. Objective: To describe the functional neurologic status of patients who underwent microsurgery for paraclinoid aneurysms at Hospital Nacional Dos de Mayo from 2018 to 2021. Material and Methods: This is a case series study. We retrospectively reviewed the clinical charts of patients with paraclinoid aneurysms to describe the functional neurologic status, measured by the modified Rankin scale (mRS), after microsurgery in the aforementioned time and hospital. Results: 14 patients received microsurgical treatment for paraclinoid aneurysms during such period of time. Previous to discharge, 57.1% of patients had a mRS grade 1 and one patient died. At 1 month, 61.5% had a mRS grade 1. At 3 months, 53.8% had a mRS grade 0 and at 6 months, 61.5% had a mRS grade 0. Conclusions: The functional status in 84.6% of patients who underwent microsurgical treatment of paraclinoid aneurysms at Hospital Nacional Dos de Mayo between 2018 to 2021 improved during the follow-up, reaching an asymptomatic stage at 6 months of discharge in 61.5% of patients.
