The Maternal and Neonatal Health Budget Program (SMN in its Spanish acronym) is the main public intervention focused on the reduction of maternal and neonatal mortality with a significant proportion of economic resources allocated to it. However, there are regions such as Piura that still have high mortality rates well above the national average. In this context, this study evaluated the efficiency of the budget execution of the SMN Program in Piura region from 2012 to 2019 to promote transparency in the management of public resources, provide information on the performance of the program and facilitate decision-making for people in charge of its execution. To that end, a retrospective, cross-sectional and case evaluation study was carried out using the methodology established in the General Guidelines for the Ex-Post Evaluation of Public Investment Projects (MEF/JICA); to establish efficiency, three indicators were calculated: efficiency in achieving products, cost efficiency, and global efficiency. In addition, through documentary review, aspects that could facilitate or limit the efficiency of the program in Piura region were identified. The results show that the efficiency of the program was mainly low, except for the products "Care for pregnant women with complications" and “Population accesses counseling services in SRH” that presented optimal performance. The constant change of authorities at the regional and national levels could have limited the efficiency of the program in Piura region, while the transition from the MDGs to the SDGs and the involvement of civil society organizations could have favored its performance.
