During gestation there is no differential diagnosis for the determination of iron deficiency anemia and a compensatory physiological process called hemodilution. Anemia on the one hand is a condition of hematological alteration, where the requirement and absorption of iron is not supplied by the pregnant woman; while on the other hand, it occurs to prevent the utero-placental flow from being affected. The present study aims to evaluate the differential diagnosis of iron deficiency anemia (IDA) and hemodilution using the hematological and corpuscular parameters of hemoglobin (Hb), mean corpuscular haemoglobin (MCH), corpuscular hemoglobin concentration (Hb), mean corpuscular hemoglobin concentration (MCH) and hemoglobin concentration (Hb), mean corpuscular hemoglobin concentration (MCHC), mean corpuscular volume (MCV) and the erythrocyte distribution coefficient of variation (RDW-CV) obtained from the automated hemogram in 348 pregnant women attended at the National Maternal and Perinatal Institute of Lima. Serum IL-6 levels were also evaluated as an inflammatory marker. As markers of iron and erythropoietic status, serum levels of ferritin (F), soluble transferrin receptor (rsTf), hepcidin, erythropoietin, testosterone and oestradiol were evaluated. According to the results obtained, iron deficiency anemia is considered when hemoglobin (Hb<11 or 10. 5g/dL), Mean Corpuscular Hemoglobin (MCH<27pg), Mean Corpuscular Hemoglobin Concentration (MCHC<30g/100mL), Mean Corpuscular Volume (MCV<84fL) and Erythrocyte Distribution Width (RDW-CV>15%), and hemodilution anemia when, except for Hb <11 g/dL, the other 4 hematological parameters are normal. Pregnant women classified as true anemic had higher values of high Soluble Transferrin Receptor (6.01±1.51) and Erythropoietin (21.33±2.61), while Hepcidin (3.65±0.17) was lower than in the group of pregnant women with hamodilution anemia, which showed a similar behaviour to the reference group (normal hemoglobin). These results were corroborated by the area under the ROC curve. In conclusion, using the automated haemogram it is possible to make a differential diagnosis of hemodilution anemia from true anaemia.
