Introduction: A prolonged excretion of Sapovirus and a high level of viral load in stool can be an obstacle for the control of infantile gastroenteritis in the community. The aim of this study was to estimate the duration and characterize the evolution of the viral load level of episodes of Sapovirus excretion in feces of children under two years of age. Methods: For the determination of the excretion period, the presence of Sapovirus was detected by reverse transcription followed by quantitative PCR (RT-qPCR) of the binding region of the genes encoding the polymerase and the capsid protein VP1 in a subset (n=440) of weekly stool samples collected before and after Sapovirus infections. Sapovirus infections were previously reported from the study cohort of newborn children in the Pampas community of San Juan de Miraflores, Lima - Peru. Likewise, genotyping of the Sapovirus genogroup was performed by amplification and sequencing of the gene encoding the capsid protein VP1 to confirm that the episode belonged to the same genotype. Results: The median duration of Sapovirus excretion episodes without diarrhea was 20.5 days (n = 15; [RIQ], 7.5-25); while Sapovirus excretion episodes with diarrhea had a median of 16.5 days (n = 41; [RIQ], 12.5-22), with no significant differences between groups (p>0.05). The duration of Sapovirus excretion showed no relationship with the age of the individual, the presence of diarrhea in the episode, the genogroup of the virus, nor with the initial viral load (p>0.05). In addition, a decrease in the initial viral load of Sapovirus shedding episodes was observed in infected children older than 10 months of age, and an evolution of the viral load in the course of shedding episodes with a tendency to reduce with time. Conclusion: The present study found a period of excretion of Sapovirus in feces of 18 days in children under 2 years of age. These findings are essential information to propose better surveillance and control measures for childhood gastroenteritis caused by Sapovirus in Peru.
