Peru is a hyperendemic country for Fasciola hepatica infection. Triclabendazole (TCBZ) is the drug of choice for liver flukes, although in the last years the cases of drug-resistant flukes in humans and animal have increased, especially in Cajamarca. Recent reports inform of TCBZ traces in meat and dairy products. Surveys were applied to assess the practices and customs of dairy farmers and TCBZ providers in Cajamarca, which could explain the presence of TCBZ metabolites in fresh milk and how they transfer to humans. In addition, the Rapid Sedimentation Technique (RST), and Kato-Katz technique were employed, confirming the presence of Fasciola hepatica eggs in cows under TCBZ treatment. Single doses of TCBZ measured by guesswork were not efficient for the control and treatment of cows infected with Fasciola hepatica. In addition, it was also determined that Triclabendazole Sulfoxide (TCBZSO) and Triclabendazole Sulfone (TCBZSO2), at an approximate withdrawal time of 20 days, were present in raw cow milk at a minimum quantifiable level of 0.006 ug/mL and 0.015 ug/mL, respectively; TCBZ was not detected at this withdrawal time by High-Performance Liquid Chromatography with Diode-Array Detection (HPLC-DAD). New control systems from the competent health authorities are required since no withdrawal time and maximum residue limit (MRL) of TCBZ and its metabolites have been established at national level to guarantee the safety of dairy products.
