Abstract: Antecedentes: La exposición al tabaco sigue siendo un problema de salud pública importante, especialmente para las mujeres embarazadas. Entre otros efectos, aumenta el riesgo de partos prematuros, de neonatos con bajo peso al nacer y pequeños para la edad gestacional. Muchos países han promulgado políticas públicas para frenar la exposición al tabaco, por ejemplo, prohibiendo fumar en lugares públicos. En el 2006 y 2010, el Perú promulgó leyes contra el tabaco que prohíben fumar en lugares públicos, requieren el uso de frases e imágenes de prevención en productos y publicidad del tabaco, y prohíben la venta a menores de edad. El objetivo del presente estudio es evaluar si las regulaciones en la legislación antitabaco aprobadas en el Perú en el año 2008 se asocian a un cambio del peso en los recién nacidos, al riesgo de prematuridad y al riesgo de ser pequeño para la edad gestacional (PEG) Metodología: Estudio cuasi-experimental que utiliza datos del Sistema de Registro del Certificado de Nacido Vivo. Los hijos de madres que viven en áreas urbanas fueron el grupo de intervención (afectados por la legislación), mientras que los hijos de madres que viven en áreas rurales se consideraron el grupo de comparación (sin legislación). En el estudio solo se incluyeron nacimientos únicos con información de peso al nacer y edad gestacional, nacidos de madres de 12 a 49 años. Se excluyeron los recién nacidos con pesos al nacer superiores a +4 desviaciones estándar (DE) o inferiores a -4 DE del peso promedio por cada edad gestacional. Para medir la asociación de la legislación sobre el peso al nacer, se realizó un análisis de diferencias en diferencias. Resultados: Un total de 2,030,774 nacimientos fueron incluidos en el análisis. Después de ajustar por las características de la madre y el niño, y las variables contextuales, la implementación de la ley antitabaco en el Perú mostró asociación con una reducción de 30 casos por cada 10,000 nacidos vivos (IC del 95%: 19 a 42) en la incidencia de prematuridad, y con una reducción de 14 casos por cada 10,000 nacidos vivos (IC del 95%: 0.40 a 28) en la frecuencia de recién nacidos PEG. Sin embargo, la política no se asoció con un cambio en el peso al nacer (cambio promedio: -2.62 gramos, IC 95%: -6.10 a +0.85). Conclusiones: En concordancia con estudios realizados en otros países, la ley antitabaco en el Perú se asoció con una modesta reducción en la proporción de prematuridad y de nacidos PEG, sin ninguna asociación con el peso al nacer.

Among other effects, it increases the risk of premature labor, low birth weight and small size for gestational age. Many countries have enacted public policies to curb tobacco exposure, for example by prohibiting smoking in public places. In 2006 and 2010 Peru enacted anti-tobacco laws that forbid smoking in public places, require the inclusion of prevention text and images in products and publicity, and prohibit sale to minors. The objective of this study was to assess whether the regulations related to the anti-tobacco legislation approved in Peru in 2008 were associated with a change in the birth weight, the risk of prematurity, and the risk of being small for gestational age (SGA). Methodology: A quasi-experimental study using data from the Peruvian Live Birth Registry was conducted. Children born to mothers residing in urban areas were the intervention group (impacted by the legislation), while children born to mothers residing in rural areas were considered the comparison group. Only singletons with information on birth weight and gestational age, born to mothers aged 12 to 49 years were included in the study. Newborns with birth weights greater than +4 standard deviations (SD) or less than -4 SD from the gestational age-specific mean were excluded. To measure the effect of legislation on birth weight a difference in differences analysis was performed. Results: A total of 2,029,975 births were included in the analysis. After adjusting for characteristics of the mother and the child, and for contextual variables, the implementation of the anti-tobacco law in Peru was associated with a reduction of prematurity incidence by 30 cases per 10,000 live births (95% CI: 19 to 42) and of SGA by 14 cases per 10,000 live births (95% CI: 0.40 to 28).
