Introduction: Microsatellite instability (MSI) is one of the 4 molecular subtypes of gastric cancer. The diagnosis can be made by immunohistochemistry, which offers adequate sensitivity and specificity. Objectives: To detect MSI by immunohistochemistry in gastrectomy samples from patients with gastric carcinoma in the pathology department of a III-1 hospital, Lima-Peru. To evaluate its association with age, sex, survival, TNM staging and histopathological and Lauren classification. Describe the clinicopathological features of MSI. Materials and methods: The study design was analytical cross-sectional. Immunohistochemistry for MLH1, MSH2, MSH6 and PMS2 was performed on all samples. Frequencies and percentages and relationships were calculated for all categoric variables and associations with MSI were sought using prevalence odds ratio and the Chi-squared test. Continuous data was analyzed with means and SD. Survival analysis was performed using the Kaplan-Meier method. Results: Of the 51 samples validated, 15 (29.4%) cases of MSI were found. The relationship between MSI and the clinicopathological characteristics studied: age, sex, Lauren classification, TNM staging, histopathological grade and survival, were not statistically significant (p>0.05). Prevalence of MSI was found in the antropyloric location (85.7%), Borrman type 3 classification (79.6%), intestinal type (86%), well and moderately differentiated (73.4%) and tubular type (80%). Conclusions: A 29.4% prevalence of MSI was found among patients with cancer gastric who had a gastrectomy. No significant relationship was found between the presence of MSI and the variables studied.
