Background: It is known that elderly TB patients are more likely to have a poor response to treatment and this may be due to multiple factors associated with age, making them a vulnerable population. There is very limited evidence about the response to treatment among elderly patients in Peru and the factors that would be associated with this response. Objective: To compare the response to antituberculosis treatment among adult (18-59 years) and elderly (≥60 years) patients with tuberculosis treated in the National TB Program at Metropolitan Lima in the period between 2015-2016. Methods and Materials: Retrospective cohort using a database of 19 406 patients provided by the Tuberculosis Directorial Information System (SIGTB) of the Ministry of Health treated in the National TB Program at Metropolitan Lima during the period between 2015-2016. A Generalized Linear Model (Poisson Regression) adjusted to potential confounders was performed to assess whether the age group is an independent risk factor for response to treatment in our study population. Results: It was found that 12 600 (86.74%) patients were adults (18-59 years) and 1 927 (13.26%) were elderly (≥60 years). In the multivariate analysis, it was found that the age group is an independent factor for the poor response to treatment in patients with pulmonary tuberculosis [RR: 1.19 (95% IC 1.09 – 1.30); p < 0.001]. Conclusion: This study showed that elderly patients with tuberculosis in Metropolitan Lima had a higher risk of treatment failure in the period between 2015-2016.
