Objective: to determine the relationship between self-care capacity and adherence to antiretroviral treatment in people living with HIV in a hospital in Lima, 2020. Material and methods: quantitative study, correlational type, cross-sectional. Made up of 219 adults between 18 to 59 years 11 months and 29 days, with antiretroviral treatment for no less than three months. The Self-Care Agency Appreciation Scale survey was used, validated by binomial test and pilot test, which showed Cronbach's Alpha greater than 0.7 and the Treatment Adherence Assessment Questionnaire. Results: It was found that 49.8% presented a good level of self-care capacity, while 48.4% presented regular and only 1.8% had low self-care capacity. Regarding adherence to antiretroviral treatment, the majority showed an insufficient level of adherence with 51.1%, followed by 25.6% with an adequate level, 22.8% presented low, while only 0.5% had a strict level of adherence. Spearman's Rho correlation coefficient (R = 0.518) shows a significant correlation (p = 0.000), direct and of a medium level, that is, high levels of self-care, correspond to high levels of adherence, and vice versa. Conclusions: A direct relationship was determined between the two variables of the study, it was found that the capacity for self-care was at a good level while adherence to treatment was insufficient.
