Introduction: Pancreatic fistula (PF) is a common complication following Whipple procedure, a major surgery performed for pancreatic cancer or other pancreatic disorders (1). Despite advancements in surgical techniques, PF remains a significant challenge, with an incidence ranging from 1% to 36% (2). Pancreatic cancer is the fifth leading cause of cancer death worldwide, and Whipple surgery is often the only curative treatment option. This study aims to investigate the association between the timing of Whipple surgery during the SARS-CoV-2 pandemic and the incidence of postoperative PF in patients at Edgardo Rebagliati Martins Hospital in Lima, Peru, between March 2018 and March 2022. Methodology: A retrospective cohort study was conducted involving a comprehensive review of medical records from March 2018 to March 2022. The exposure variable was the period of surgery (pre-pandemic or during the pandemic), and the outcome variable was the development of postoperative PF. Descriptive and analytical statistics were employed. Results: A total of 200 medical records were analyzed (100 per period). The most prevalent histological type was adenocarcinoma of the Vater ampulla (36.5%). Among the complications, PF was the most notable finding (pre-pandemic:43% and during the pandemic: 18%). In the multivariate model, PF was not associated with the period of surgery (p=0.415) but was significantly associated with prolonged hospitalization (p=0.029) and the need for intraoperative transfusion (p=0.006). Discussion: Despite the observed difference in PF rates between the pre-pandemic and pandemic periods, this association was not statistically significant. This could be attributed to the inherent characteristics of Whipple surgery and may require further investigation in other patient populations and pathologies. Conclusion: No significant association was found between the development of PF and the timing of Whipple surgery during the pre-pandemic and SARS-CoV-2 pandemic periods.
