ABSTRACT The purpose of this research work was to determine the effect of a pharmaceutical intervention on the level of therapeutic adherence of hypertensive patients in a health center in Trujillo. A quasi-experimental before-after study design was used with a single group, which consisted of a total of 19 hypertensive patients who were treated at the health center, who were selected taking into account relevant inclusion and exclusion criteria. After intervention (before) and after pharmaceutical intervention (after) the level of adherence was measured with the Morisky test in its version of 8 items. Statistical processing was performed using frequencies and percentages presented in univariate and contingency tables; the significance of the effect was determined by the Mc Nemar test. The results showed that before the pharmaceutical intervention the level of low adherence predominated with 36.84%; while after the intervention, the level of low adherence was reduced to 15.79% and the level of high and medium adherence prevailed with the same frequency equal to 42.11%, with a p = 0.049. It is concluded that the pharmaceutical intervention had a significant improvement effect on the level of therapeutic adherence of hypertensive patients
