The main objective of this work is to analyze the dosimetry of treatment plans in the pulmonary region with Volumetric Modulated Intensity Arcotherapy (VMAT), by using ArcCheck diodes and radiochromic EBT3 films to extend the application of this technique combined with the field technique in field (FIF) to procedures of total body irradiation (TBI), in such a way it is possible to reduce doses to the lungs and other organs at risk, in addition to meet certain conditions of simplicity, repetitiveness, comfortable positioning of the patient, ho- mogeneity of the dose be less than 10 % for which the Report No. 17 of the AAPM was used. The treatment uses photon beam produced by linear accelerators (6 MV) and absorbed dose values (\ thickapprox12 Gy in 6 fractions). To guarantee the consistency between the clinical prescription of absorbed dose and its administration to the patient, it is necessary to perform the calibration and characterization of the dosimetry objects, the ionization chamber (IC), matrix of 1386 diodes (ArcCheck), radiochromic films (EBT3). and Epson 11000XL scanner, dose optimization is performed with the Analytical Anisotropic Algorithm (AAA) incorporated in the treatment planning system Eclipse 13.5 (Varian Medical Systems). The translations of ArcCheck and EBT3 films for the 10 cases of the study were carried out with an approval rate (> 95 %) with a global gamma analysis of 3 % and 3mm. The measurements with diodes (99 % on average) have a better approval rate with respect to the system with films (97,8 % on average). TBI with VMAT in the lung region combined with the FIF technique in the other regions of the body, provides the benefit of a satisfactory dose distribution within the PTV (D95 ≥ 95 % and D2 ≤ 107 % of the prescribed dose), while selectively reducing the dose to the lungs, and if necessary, in other organs. However, the planning time is extensive (4 hours on average). It is concluded that the VMAT technique is appropriate to perform the treatments of the TBI in the lung region combined with the technique of subfields in the other regions of the patient’s body.
