In this research the problem of fatalities in mining operations at the country level, continent and the world is reviewed; as scientific literature is also reviewed with the most recent contributions related to safety engineering and risk management. The purpose of this paper is to show that through the management of critical safety risks can be prevented fatalities in underground mining operations. As an added value provides a theoretical framework that can serve as a reference for students, teachers and researchers. Such research was conducted in the operations of a mining contractor in an underground mine in the period 2008- 2017. For this purpose the mapping processes used technique concentration of hazards,09 critical risk. From these critical risk management critical controls, the International Council on Mining & Metal, the BHP Billiton critical controls decision tree, the principle ALARP and Bowtie methodology was applied to determine the necessary critical controls to ensure that risks are tolerable range in establishing preventive and mitigating controls. Data analysis for the period 2008-2012 (before) and during the implementation period 2013-2017 was taken. As a result,no fatalities were recorded in the second period and safety statistics improved positively reducing frecuency rate 30%. Critical risk management demanded the implementation of essential critical controls. These controls represented an investment of $12,095,084.27 additional in the annual budget. Regarding administrative fines for security; these sanctions were reduced to zero and the hypothesis was demonstrated with the Student t test. Finally, it was shown that the management of critical security risks contributes to the prevention of fatalities in underground mine.
