Cambio en las funciones estructurales del self de una paciente con depresión en una terapia psicoanalítica breve

Abstract

La presente investigación es un estudio sistemático de caso único de una paciente con depresión en el curso de un proceso de psicoterapia psicoanalítica breve. Se tuvo como objetivo evaluar el cambio en las funciones estructurales del self en la fase inicial y final del tratamiento utilizando el Eje IV del OPD 2. Este instrumento permitió una descripción cualitativa profunda y dinámica de las funciones estructurales en ambas fases del proceso a partir de la evaluación de las capacidades, subcapacidades y dimensiones estructurales de la consultante. Los resultados encontrados evidenciaron cambios hacia el final del tratamiento en el funcionamiento global de las capacidades cognitiva, de manejo y emocional los cuales son comprendidas desde los cambios en el funcionamiento de sus subcapacidades y dimensiones estructurales. Asimismo, se evidenciaron cambios hacia el final del tratamiento en el funcionamiento de la subcapacidad objeto externos perteneciente a capacidad de vínculo, la cual no mostró cambios en su funcionamiento global. Alcances y limitaciones de estos resultados son discutidos hacia el final de la investigación.

The present study is a single case and systematic study of a patient with depression during the course of a brief psychoanalytic psychotherapy process. The aim of the study is to evaluate the change in self´s structural functions between the initial phase and the final phase of the treatment using the Axis IV of the OPD 2. This instrument allowed a deep and dynamic qualitative description of the structural functions in both phases of the process from the evaluation of the patient’s structural abilities, subcapacities and dimensions. The results show, towards the end of the treatment, changes in the global functioning of cognitive ability, regulation ability and emotional ability, which are understood from its structural subcapacities and dimensions. Furthermore, towards the end of the treatment, there were changes in the functioning of attachment to external objetcts subcapacity, which belonged to the attachment ability. Implications and limitations of these results are discussed at the end of the investigation.
