This clinical case is about Mirella, an 18-year-old girl who comes to the clinic looking to unload her emotions without being judged and misundertood. The objective of this counseling space, which consisted of 5 sessions, was to try to make her question if she could really trust someone, as there were several experiences in her life that led her to psychic pain associated to the perception of lying and judgment. For the development of the analysis the work of various authors has been used, work that mentions about technical theoretical psychoanalytic approaches and some essential characteristics were considered in the analytical framework, characteristics which were made more flexible in the encounter with the patient. The type of objectal relationship that dominates her internal world is partial, since she cannot integrate that a person can have positive and negative aspects, but instead perceives her environment as very threatening and the distress that prevails is the paranoid one because her Ego is afraid of annihilating itself and expels the negative outwards. Among her defense mechanisms is splitting, projection, and repression of her emotions. Mirella has resources such as the capacity for reflection, understanding of the opinions of others and a positive attitude for the session and she could continue with psychoanalytic therapy to keep up working on the various subjects that she may bring up, but she should strengthen her therapeutic commitment, which will allow her to be capable to engage, trust and tolerate the intimacy of the process.
