Objective: The aim of this study was to compare the treatment success of onychomycosis in patients treated with Q-Switched Nd: YAG 1064 nm laser and pulsed itraconazole. Materials and methods: This was an observational, analytical retrospective cohort study, based on the review of medical records from a private dermatological center. The sample was 124 medical records of patients with onychomycosis treatment divided into two groups of 62 records, according to the treatment they had received. Then, the probability of treatment success was evaluated using generalized linear models with Poisson family. It was adjusted for the type of treatment variables and other identified confounders. Results: No statistically significant differences were observed in any of the evaluated variables between the groups. It was found that 85.4% of patients treated with laser had treatment success compared to 66.1% of patients treated with itraconazole, which was statistically significant (p=0.010). Patients treated with laser had a 27.0% higher probability of treatment success compared to those treated with itraconazole, which was statistically significant (p=0.001). Conclusions: Q-Switched Nd: YAG 1064 nm laser treatment demonstrated a higher mycological cure probability success for onychomycosis compared to pulsed itraconazole treatment.
