Objective: Acromioclavicular dislocation is a common shoulder injury that can significantly affect its functionality. This injury is classified according to the Rockwood system, ranging from types I to VI, depending on severity and the affected structures. For types I and II, conservative treatment is typically preferred. In more severe cases (types III-VI), where there is significant joint displacement, surgical intervention is often necessary to restore shoulder function and stability. Materiales and methods: A comparative study was conducted with a sample of 60 patients diagnosed with acromioclavicular dislocation. Thirty patients underwent treatment with the modified four-suture technique, while the other thirty received treatment using the hook plate technique. The Quick DASH index was used to assess functionality, and the Likert scale was employed to analyze aesthetic outcomes. Additionally, unit costs associated with both surgical techniques were calculated. Results: The modified four-suture technique demonstrated superior outcomes in terms of functionality (p = 0.037), lower incidence of late complications (p < 0.001), better aesthetic results (p = 0.003), and significantly lower costs (p < 0.001) compared to the hook plate technique. Conclusion: The modified four-suture technique proves to be more efficiency than the hook plate technique in surgically treated patients with acromioclavicular dislocation at HVLE between 2018 and 2024
