To determine the risk factors and establish a predictive model ofcomplications of zonulocapsular support in cataract surgery. MATERIAL AND METHOD: A retrospective analytical observational study with case-control design of patients operated on for cataract surgery by phacoemulsification at the Regional Institute of Ophthalmology - La Libertad between the period 2018-2019 was performed. The sample included 160 medical records, made up of 120 controls and 40 cases. Clinical and demographic data were collected to identify statistically significant variables and develop a predictive model. RESULTS: Of the 40 cases recorded, 30 were due to posterior capsular rupture and 10 to zonular dialysis. The variables that were statistically significant were previous posterior segment surgery (OR:15.29; p:0.017), 2 systemic comorbidities (OR:3.12; p:0.003), 1 ocular comorbidity (OR:3.42; p: 0.001), poor pupillary dilation (OR:8.88; p:0.005), anterior capsule fibrosis (OR: 3.35; p:0.047), nuclear opacity ≥ 4+ (OR: 5.75; p:0.004), weak red reflex (OR: 4.57; p<0.001) and absent (OR: 7.78; p:0.009). The predictive model presented a sensitivity of 37.5%, specificity 93.33%, PPV 65.22%, NPV 81.75%; and the value of the ROC curve was 0.8. CONCLUSIONS: Risk factors for presenting complications of zonulocapsular support in cataract surgery included previous posterior segment surgery, two systemic comorbidities, one ocular comorbidity, poor pupillary dilation, anterior capsule fibrosis, nuclear opacity ≥ 4+, weak and absent red reflex. The predictive model obtained an AUC that qualifies it as a good test, with high specificity and NPV
